Notes:
- McCarthy must decline to testify until the $500 testimony minimum is paid.
- McCarthy must decline to travel until a travel advance (including tickets if appropriate) is paid.
- Any unused portion of a travel advance is fully refundable.
- The travel advance is not a per diem.

Invoice Terms:
Net 30 days from date of invoice. (Overdue 1.5% per month)
